Information & Assistance
Not sure where to begin? Our Community Resources Specialist will help connect you with the services in Lake and Mendocino Counties that may help in your particular situation.
Multipurpose Senior Services Program
MSSP provides care management to persons aged 60+ that they may remain safely at home. Recipients must be eligible for Medi-Cal and certifiable for skilled nursing care.
Community Care HIV/AIDS Program
CCHAP provides serves for individuals on Medi-Cal who are diagnosed with HIV or AIDS in Lake, Mendocino, and Sonoma Counties and need help remaining independent at home.
HIV Care Program
HCP offers support services to residents of Lake and Mendocino County living with HIV (who may not meet the requirements of the CCHAP program).
Housing Opportunities for Persons Living with AIDS
HOPWA helps Lake and Mendocino County residents living with HIV maintain stable housing.
The Clearlake Drop-in Center provides counseling, social activities, informational meetings, support groups, and literature to persons living with HIV and AIDS.
Supported Living Service
SLS assists Regional Center consumers with developmental disabilities to live independently in the community and in their own homes.
Therapeutic Arts & Performance Studio
TAPS offers Redwood Coast Regional Center consumers the opportunity to express their creativity in various artistic forms.
Inclusive Senior Services
ISS provides senior Redwood Coast Regional Center consumers an opportunity to stay active, pursue new interests, and socialize.
About Community Care
Community Care is a non-profit agency serving Lake, Mendocino, and Sonoma counties. For over 40 years, our goal has remained simple: help individuals in our community with disabilities or illnesses to remain safe and independent at home.
We offer multiple programs (see above) that can provide you (or a loved one) with the information and support you may need.
Most of our programs will meet with you to learn about your needs and develop a plan for care. Our staff can also help to arrange for necessary services using community resources or direct purchase.
