CCHAP
We know it can be difficult to manage living with HIV and AIDS. The Community Care HIV/AIDS Project (CCHAP) is committed to helping you stay as healthy as possible for as long as possible.
What is CCHAP?
The Community Care HIV/AIDS Program (CCHAP) supports people of all ages living with HIV or AIDS in Mendocino, Lake, and Sonoma counties. Since 1988, CCHAP has provided medical and social work case management to help individuals remain independent, healthy, and in their homes.
Through its Medi-Cal Waiver Program, CCHAP delivers home and community-based services designed to help participants avoid institutional care, including comprehensive support and direct care services not typically covered by Medi-Cal.
CCHAP also offers education, prevention, counseling, and substance abuse counseling for the wider community.
Who’s Eligible?
To qualify, individuals must meet these criteria:
Age: All ages.
Residency within Lake, Mendocino, or Sonoma County.
Be diagnosed with HIV or AIDS.
Be Medi-Cal eligible with no share of cost.
Require a level of care comparable to a nursing facility or hospital, but choosing to live at home instead of an institution.
What services are provided by CCHAP?
Depending on your level of need, some services and benefits you may be eligible for include:
- Case Management
- Homecare and Chore Services
- In-Home Nursing Care
- Skilled Nursing and Attendant Care
- Transportation Assistance
- Legal Assistance
- Nutritional Support
- Counseling and support groups (for yourself, your friends or family)
- Adaptive Equipment
- Emergency assistance with food, transportation, housing, and utility bills.
What happens when I enroll?
CCHAP is a Case Management program. When you enroll in CCHAP, a Nurse Case Manager and a Social Work Case Manager will meet with you to learn your needs. Working together, you’ll develop a plan to help you coordinate all aspects of your life, including:
- HIV-related care
- Managing medical, financial, domestic, and emotional issues
- Providing current information about home care services and medical treatments.
- Assistance with securing Social Security and Medi-Cal benefits
- Help locating affordable housing
CCHAP clients may also qualify for financial assistance for food, transportation, housing, and utility bills.
Please note that CCHAP is not a crisis intervention service for medical, mental health, or other emergencies and cannot provide services prior to enrollment.
Who pays for CCHAP?
There are no fees for our services.
CCHAP’s waiver program is funded by the Medi-Cal Waiver Program (MCWP), which is a specialized home and community-based services waiver administered by the California Department of Health Care Services (DHCS) in collaboration with the California Department of Public Health, Office of AIDS.
CCHAP also receives funding from the Ryan White CARE Act (Part B), the Lake County Behavioral Health & Alcohol and Other Drug Services (AODS), the Redbud Health Care District, Housing Opportunities for Persons With AIDS (HOPWA), and private donations.

Ukiah: Monday – Friday 8 a.m. – 5 p.m.
Clearlake: Monday – Thursday 8 a.m. – 5 p.m.
Santa Rosa: Monday – Friday 8:30 a.m. – 5 p.m.
