We know it can be difficult to manage living with HIV. Community Care’s HIV Care Program (HCP) is here to help.

What is HCP?

Community Care has been assisting persons living with HIV/AIDS since 1988. The goal of the HIV Care Program (HCP) is to help those diagnosed with HIV to live as healthy and independent as possible, for as long as possible.

Who’s Eligible?

To qualify, individuals must meet these criteria:

  • Age: All ages. 

  • Reside in Lake or Mendocino County.

  • Be HIV positive.

  • Be low income.

What services are offered by HCP?

Some services and benefits you may be eligible for include:

  • Case management services 
  • Resource and referral assistance 
  • Food bank
  • Nutritional assistance 
  • Transportation assistance 
  • Potlucks and events to build support (Lake County)

Who pays for HCP?

There are no fees for our services.

The HIV Care Program (HCP) in California is funded primarily by the federal Ryan White HIV/AIDS Program Part B, with additional support from the Centers for Disease Control and Prevention (CDC) and the California State General Fund.

HCP is administered by the California Department of Public Health (CDPH) through the Office of AIDS.

What is the difference between CCHAP and HCP?

CCHAP is a more specialized Medi-Cal benefit for those with higher medical needs. HCP recepients do not need to be on Medi-Cal nor do they have to meet the higher medical needs that are required by CCHAP’s Waiver program.
